Is it possible to blind fire in the Matrix against an undetected Icon?
Is there a penalty to not getting hit for other attackers having attacked since the runner's last pass?

No. An attack in the Matrix is not a real melee/ranged attack. It is your code attacking other code, which is interpreted for the user (you) as a visual image of a melee/ranged attack. If your code can't locate the target code, it can't attack.
